Job Description Summary

Medcan is seeking a compassionate and skilled Social Worker to join their transformational healthcare team in downtown Toronto. This part-time role involves providing client-centered care and developing action plans to help clients live their healthiest lives.

Responsibilities

  • Review and interpret client screening results to address client profiles and self-identified goals and needs
  • Provide recommendations or referrals for ongoing treatment as appropriate
  • Leverage Medcan’s Preferred Provider Network to connect clients with specialized care
  • Implement evidence-based strategies including Cognitive Behavioral Therapy, Positive Psychology, and Motivational Interviewing
  • Develop comprehensive client action plans that address current or potential obstacles, build resiliency, and enhance performance
  • Build strong relationships with clients’ Annual Health Assessment teams to coordinate care and enhance client experience
  • Create detailed summary reports with relevant client insights and action plans, including referrals and resource recommendations
  • Support new program development through content creation, clinical research, client feedback management, and pilot program implementation
  • Regularly update client resources and clinical tools as necessary
  • Evaluate and report on external providers and community resources that Medcan clients are referred to
  • Deliver care through multiple channels including email, video, phone, and chat
