This Clinical Care Coordinator III (RN) position involves supporting physicians with patient care, health education, EMR management, and implementation of clinical protocols in Columbia University’s pediatrics department. The role requires an experienced nurse who can deliver excellent patient experiences while serving as a key member of the multidisciplinary care team.

Pediatric Nursing Clinical Coordinator Responsibilities

The Clinical Care Coordinator III position at Columbia University’s pediatrics department encompasses a wide range of responsibilities designed to ensure excellent patient care and smooth clinical operations. The successful candidate will be expected to:

Administrative and Clinical Coordination

  • Assess and collect patient health data, inputting medical information into the Electronic Medical Record (EMR) as directed by physicians
  • Develop comprehensive care plans across the healthcare continuum (inpatient, outpatient, home, skilled nursing facilities, schools)
  • Manage medication renewals and medical prior authorizations while administering immunizations and medications as needed
  • Execute nursing interventions by collaborating with the care team, following established departmental standards of care
  • Take full responsibility and accountability for effectively managing nursing care of individual patients
  • Support ongoing clinical functions to provide compassionate, patient-centered care
  • Handle telephone, MyChart, and in-person triage for symptom management and patient education
  • Respond to urgent and emergent patient situations with appropriate interventions

Healthcare Coordination Expertise

  • Coordinate multidisciplinary healthcare services across specialties
  • Arrange diagnostic tests (lab, x-ray) as ordered by physicians
  • Perform diagnostic testing, therapeutic, and interventional procedures when necessary
  • Review and act on test results from various sources
  • Manage communications from internal and external stakeholders, including in-basket messages
  • Update immunization records and health maintenance items regularly
  • Ensure compliance with infection control protocols within the practice
  • Implement appropriate measures to enhance patient and family satisfaction
  • Provide cross-coverage nursing support across all clinical areas within the Children’s Center of Excellence

Team Leadership and Mentorship

  • Participate in recruitment of clinical staff members
  • Precept clinical staff including medical assistants, licensed practical nurses, and registered nurses
  • Conduct competency testing for both new and established clinical staff
  • Serve as a mentor and resource to colleagues and team members
  • Supervise and monitor regulatory compliance items, including emergency medication inventory and vaccine storage
  • Track vaccines according to approved internal protocols
